Pivot Chart Form does not display in Report

I have created a Pivot Chart form from a Query. Everything works inside of the form but when I drag it to my report it does not display any info. It actually does not seem to be connected to the query when I drag it to the report. The filter dropdowns are blank. Am I missing a step?

Note that MS has removed creating pivot tables/charts from Access 2013.

I was hoping for some more insight on the matter. I followed those steps the first time. I will just have to keep messing with it. Thanks.

I've never needed pivot table/chart. If you want to provide db for analysis, follow instructions at bottom of my post.
