This is why only way I have been able to find to do this.
Create a new blank Access database. This will be your template "storage" dB. (I'll call it the Template dB)
In your current dB (I'll call this dB the Working dB), set up the "look and feel" of the form the way you want. Save the form. (I'll call this form the Template Form)
Close the "Working" dB and open the "Template" dB. Click on the "External Data" menu. In the group "Import and Link", click on "Access" and follow the steps to import the "template form" from the "Working" dB.
Done.
So now you are in the "Working" dB, and you want to add a new form.
Click on the "External Data" menu. In the group "Import and Link", click on "Access" and follow the steps to import the "template form" from the "Template" dB.
You can use the same method to have a Report template.
OK, now how do I disconnect the fields from the original table used to create the form and insert new fields for the new table?
Delete/change the record source?
You can put unbound text boxes, text boxes, etc as place holders, but obviously the record source can/will change for each use of the form.
You would still have to connect the "Template" form to a record source
The template part is to have the same colors, (continuous/single) view, basic buttons (like a close form button and code) across multiple form.

You do realize this thread is from almost 2 years ago??? By posting in this thread, you have done what is termed "Hijacking a thread". It is better if you start your own thread, ask a question and reference this thread......
You will get more responses by starting your own thread, than posting in a thread marked solved from 2 years ago...
But, yes, is should also work in A2013.

Why not just use copy/paste ? creates a copy of the old form and asks for a new form name !
