Filter a form using a listbox

I've tried several ways of doing what I wanted to accomplish, but none of what I could find in Google/youtube has gotten me the result I need.

I have a form that I'd like to have filtered by a listbox. My main problem is that I need it to filter based on a name as well as what will be selected in a list box.

So, for example, when the form loads, I'd like it to filter by the name Sally just to pull all records associated with her. From there, I have my 1st listbox that has the "quarter" (Q1, Q2, etc.). The last list box will update based on the selection made in 1st listbox (i.e. Q1 is selected and dates 2/1 and 2/2 are given in the 2nd listbox).

Finally, I'd like the form to further filter Sally's records based only on what date is selected in the 2nd list box. So, for example: Sally has records from Q1 and Q2. I selected Q1, and I am given 2/1 and 2/2 in the second list box to choose from. I choose 2/1, the form filters for that record.

    How can I accomplish this?

Thank you for the reply; I am unable to access the file. I tried opening, but get a message that the database is in an unrecognized format. I am using Access 2007, so I'm unsure of what could be causing this. I tried a suggestion to compact/repair the database, but I still receive the same error.

Sorry for the late reply, I have been on vacation.
