Access entry form showing duplicate data for new entry

Please I need help resolving this issue. I am trying to create a new record for new employee, using the database entry form.

The access database has interface with HR system and the HR feeds and updates the Access database Employee table. We have employees record in the Access database. The database is used to create employees authorities to purchase or buy items.

I tried to create a new employee authority by selecting the employee name in the lookup field, and when I selected the name the entry form showed duplicate data for each field in the form.

    Examples:

    Authority: Authority for consulting services
    Authority for consulting services

    Department: Information Services

    Information Services

This happens for every new entry I tried to create. I cannot create new entries now because of this issue. Please I need help to stop this then I can create my new entries.

    Thanks,
    Mike

I can see that the image matches your description, but I don't know how your creating the output result as seen.
without seeing the source of the problem I cannot offer a solution.

Trying to diagnose your problem from a screen shot is like asking a mechanic to look at a picture of your car and tell you why it keeps stalling out! As Trevor suggested, we need to be able to 'look under the hood!' And if you save your file as an MDB file, more of us will be able to have a look at it and possibly help you.
