Setting up Users, Passwords, and Permissions

I've never done this before on a database and have limited experience with databases overall.

I want to setup custom user names and passwords (in other words, not the windows user names) for my coworkers with various levels of access to the database as follows:

Level 1 - access to reports only
Level 2 - access to reports and some forms
    Level 3 - full access

I remember finding an example file a couple years ago, but could never get it to work; and I can't even find that now. When I search, all I find is how to use Windows User ID. Can anyone help me with this?
