Form to enter/view data, 5 fully normalized, related tables with test data

I need help with a form to view/enter data. The database has five fully-normalized, related tables (relationships.png).

FormMockUp.pdf is a mockup of what I want the form to look like. frmMain.PNG is a screen capture of my failed attempt.

Here are screen captures of the tables, populated with a little test data:
    tblGenres.png, tblLicks.png, tblLickSorts.png, tblRockLicks.png, tblSorts.png.

    The database itself is app_07-14-12.mdb.

Please help me create the form like FormMockUp.pdf.

    Thanks,Click image for larger version.

    Hi -

What is the main form supposed to do? Is it for data entry, or data display? Specifically, what is the subform sfmLicks being used for? All it has is lists of other entries in the Licks table (if it was meant to indicate the genre if each Liscks entry, then you need a combo box, not a subform).

    Can you clarify, please?
