Hello,
I created a form that I enter criteria for a report. The query will not display the date field, yet if allows me to use it in a IIF statement. I need to display it in the report. I have spent lots of time trying to figure out if I missed something in the creation of the form, or if I am not retrieving it in the correct manner. To retrieve it I am using FromDate: [forms]! [PymtDueRpt]![FromDate]. FromDate is the name in the form.

That should work.
Check to make sure you are spelling the form name and field name correctly. Try it in the debug window to verify.
